- Where is Allergy Associates of Western Michigan clinic located?
- They have an office just down East Beltline near
Meijers but across the Beltline. They also have an office in Grandville that
may be more convenient for patients. Please check their website www.allergywestmi.com
for additional information.
- Does Allergy Associates take my insurance?
- They participate with most insurances so most
likely YES to the same ones we participate with. There may be rare
exceptions. This will be confirmed when you contact their office to set up your
first appointment with Allergy Associates.
- Do I need to fill out a transfer of records form
for my records to go to Allergy Associates? NO.
- This is only needed if you would like to have
your records transferred elsewhere. Please note Labwork done via one of the
major local hospital networks should already be available via your MyChart or
similar apps. In addition, Dr Dubravec
has routinely sent his reports to patients’ primary care providers so they
should already have records on hand. If you would like records sent elsewhere,
we ask that we receive a signed authorization
form, typically faxed from the receiving office, by 30 June 2025 to allow for
processing. After 8 July 2025 all records will be with Allergy
Associates of Western Michigan (616) 531-6900, (3185 Macatawa SW Ste B,
Grandville, MI 49418).
- How do I contact Allergy Associates of Western Michigan to
schedule my first appointment there?
- Allergy Associates’ website www.allergywestmi.com
has contact information to schedule an appointment with them, including a
patient portal. We are working together to ensure a smooth transition. Due to
the number of patients contacting their office for a first appointment, please
know that it may be 1-2 business day before hearing back from their office at
this time.
